Diagnostic & Treatment Consultation: Once a consulation is purchased ($50.00 fee), a medical questionaire will be emailed to you to fill out. You are able to write as much as you need to. Dr. Schwartz will review the completed questionnaire and will return a complete (1-2 page) description of the possible problems and all the possible diagnostic techniques available to accurately diagnose the problem. He will also include a full therapeutic course of treatment.
By purchasing a consultation, you are agreeing to the terms stated above and will recieve a printable version of the consultation within 48 hours.
